West Side United is excited to announce that our first grant is now available through a competitive RFP process! The attached RFP is in accordance with West Side United’s 2018 strategy and goals. The work of West Side United is focused on addressing the social determinants of health – economic vitality, education, the neighborhood environment, and health care –to improve health outcomes.
In the Fall of 2017, a 16-person Planning Committee made up of residents and non-profits serving the West Side was established to identify the initial set of projects for the collaborative. After reviewing neighborhood-level data to identify gaps, prioritizing areas of focus, and evaluating potential high-impact projects, the Planning Committee identified Expansion of Community Health Workers as a priority for 2018.
Community Health Workers play a critical role in supporting better chronic disease management, linking patients to resources, and promoting preventative care. Currently, many Community Health Worker models exist. The intent of this RFP is to leverage and support existing programs to expand the number of Community Health Workers on the West Side.
